PROVIDING OPPORTUNITY FOR COMMUNITY SUPPORT

ROSS LAUNCHES COMMUNITY CAMPAIGN WITH $50,000 GIFT---Mike Ross, center, recently showed his support of the Davis Health System Foundation’s Davis House project, which will provide a home away from home for cancer patients on the DMH campus. The Foundation has initially created six distinct types of funds. Each fund has, within its purpose, a particular aspect of Davis Health System. The Foundation Board may create additional funds as it determines necessary. The original funds with corresponding descriptions are as follows:

General Fund
Donations to this fund can be made at any time and in any amount. Donations received through annual giving campaigns and special events will be placed in the General Fund. Unless otherwise specified in the appeal, donations to the General Fund may be used to support any purpose consistent with the mission of the Foundation. No more than 50% of the donations to this fund in a calendar year may be used to fund the operating costs of the Foundation. However, General Funds may be used to help support one or more of the other field of Interest funds.

Hospital Bed Replacement Fund - Hospital beds are one of the most vital pieces of equipment used in healthcare, determining patient comfort and containing many features that assist in the treatment of various conditions. This project hopes to fund the replacement of three or more beds demonstrating Davis Health System's ongoing commitment to providing the best possible care for its patients.

Health and Wellness Fund
This fund is designed to promote health and wellness initiatives within the DHS service area. These initiatives may be sponsored wholly by DHS or in collaboration with other organizations. Examples of activities or events supported through this fund may include but are not limited to flu vaccine clinics, health fairs, and sports physicals.

Cancer Center Patient Assistance Fund - A sub-fund created in 2009 by an initial donor. Monies in the fund can be used to help with travel and lodging expenses and also medication costs associated with treating the side affects of cancer treatments. Anyone may contribute to the fund. General Patient Assistance Fund - The Patient Assistance Fund is designed to be a resource to patients and their families. It will offer assistance to those who have no other recourse in obtaining medication, temporary lodging, or travel and meals expenses needed to obtain necessary medical services.

Capital Projects Fund
The focus of this fund is essentially on land, facilities, and equipment. Included within that scope are facility replacement, renovation, new construction, and acquisition. The purchase of certain types of equipment used for the diagnosis and treatment of disease may also be supported by this fund.

Public Relations and Promotional Fund
This fund can be used to designate financial support for signage, seasonal décor, landscaping, etc. It may also support “image building” initiatives, promotional and marketing campaigns throughout Davis Health System.

Technology Fund
Within this fund are projects that use the latest technology in the areas of information services such as electronic medical records, (EMR). Initiatives in the areas of telemedicine, robotics, virtual surgery, etc. may also be funded through the Technology Fund.

Telemedicine Initiative - Access to comprehensive healthcare services is central to our health and well-being. Advancing technology enables various diagnostic and treatment procedures to be performed in locations different from where the actual clinician may be loacated.

Much of Davis Health System's five county service area is rural by nature. Long distances and extreme winter weather conditions coupled with the mountainous terrain create difficult travel conditions. In addition, valuable time is spent traveling to distant parts of the service are-time that can be decreased through the convenience of telemedicine. The current economic climate and the overall emphasis on reducing health care costs also support the benefits of telemedicine.

This initiative will fund the creation of a specific plan to implement telemedicine within Davis Health System

Memorial Fund
This fund has been established to enable donors to honor and recognize individuals of their choice. Those honored or recognized do not have to have a prior association with Davis Health System. The Memorial Fund will be used for scholarships to help further the skills and education of those working within Davis Health System and those residing within our service area. Of special emphasis are those skills, degrees, licenses and/or certifications that are in high demand to meet the health and wellness needs of residents within the service area. Note:
Within any of the general fields of interest remains the opportunity for a donor(s) to create a “named” fund. There are specific requirement in order to create a “named” fund. These are contained in the fund description. The initial donation must be of a certain amount to initiate the fund. Also within a specified period of time, the fund must reach a certain level to be established. A named fund may be structured to narrowly define the use of that particular fund.
